As a result of COVID-19, the New York State Department of Labor (NYSDOL) has emphasized adherence to a regulation that directs employers to provide employer identification information and NYSDOL - Record of Employment (IA12.3) form below to team members that separate from the organization; see attached. Completing the form requires campus Federal Employer Identification and New York State Employer Registration Numbers. That information has been attached. Refer to the campus you were employed with before separation. Once completed, the form should be sent to the NYSDOL for processing. Please be advised that in providing this information, NewYork-Presbyterian is not deciding on an individual’s eligibility to receive unemployment benefits. The IUI (unemployment) benefits claims center will make that determination.
